Initial Contact/ Assessment: One of our compassionate and knowledgeable placement specialists will gather information regarding the level of care needed, geographical desirability/location, physical, emotional and social needs as well as financial budget.
Placement Recommendations: Our placement specialist will compile a selection of optimal senior living recommendations based on the initial assessment which might include independent living, assisted living and assisted living homes.
Guided Tours: Our placement specialist will provide guided tours of the selected facilities. During these tours, you will be able to meet the owners, staff and managers and ask any questions you may have.
Selection and Move-In: After touring facilities and reviewing all options with you, the placement specialist will help you select the right choice. We will take care of all the red tape (including insurance paperwork and coordinating with the facility/home) so that you or your loved one can focus on getting settled into your new home.
Follow-Up: After you or your loved one is moved in, Aging Adult Care Placement Services will still be on your side. We are not satisfied with your placement unless you are—we will make sure that the home we chose together delivers everything that was promised and meets all your needs.
Future Support: If your level of care needs change, Aging Adult Care Placement Services will be here to help you reevaluate your options and seamlessly transition to another community. We have no affiliations or financial ties with any facility.
Can we keep the same doctor?
You may continue seeing your doctor. If at any time you find it difficult to get to the appointments, there is always an option of having a mobile physician come to you where you are living.
