Strategic Partnership to Reduce Unnecessary Hospital Readmissions and Improve Patient Outcomes

At Aging Adult Care Placement Services, we are dedicated to identifying the most appropriate care facility for each individual patient post discharge from a hospital or skilled nursing facility. We take pride in providing personalized care for each of our senior clients and ensuring their needs are met with the utmost respect and dignity. We partner with hospitals, skilled nursing facilities and a variety of care facilities in the Greater Phoenix area to achieve this objective. The Centers for Medicare and Medicaid Services (CMS) created the 30-day readmission measures to encourage hospitals and other health providers to evaluate the quality of care given to patients and to exercise more caution when discharging patients.


Care transitions occur when a patient moves from one health care provider or setting to another. When people living with serious and complex illnesses move from the hospital to home or a nursing home, they may be at risk for readmission back to the hospital if they develop a complication. These complications are often preventable. Nearly one in five Medicare patients discharged from the hospital—approximately 2.6 million seniors—is readmitted within 30 days, at a cost of over $26 billion every year.  (Source: CMS.gov)


There are multiple factors involved in hospital or skilled nursing facility readmissions. Hospitals and skilled nursing facilities have oversight and  responsibility for some of these factors while the patient is under their care. However, there are other contributing factors that are beyond their control once the patient leaves the hospital or skilled nursing facility. We believe correctly identifying and correcting key patterns post-discharge is critical to implementing preventative measures to reduce readmissions.


Aging Adult Care Placement Services provide placement guidance, support and resources to patients upon discharge from the hospital or skilled nursing facility. We provide hospitals and treatment facilities with report outcomes. Working together we can improve the overall quality of care for the patient while reducing costs for the hospital or skilled nursing facility due to unnecessary readmissions.
